Primary Care Access Service

On the 1st June 2019, The Southern Hampshire Primary Care Alliance went live with new streamlined service for patients in the Fareham and Gosport and South East Hampshire CCG areas. This service provides pre-bookable GP extended access appointments as well as urgent out of hours GP and Acute Practitioner appointments. What this means is greater availability of appointments outside of core working hours in order to address the health needs of the general public particularly useful for workers with little flexibility.

In addition, the out of hours GP and Acute Practitioner service accessed via 111 means that patients can avoid going to the Emergency Department and be managed in the community as and when appropriate. The integrated nature of the service ensures that if a patient has been assessed by the 111 Service as medically unable to attend a hub, an out of hours home visit can be arranged as an alternative to a hospital visit.
