New Patient Registration

If you live in our practice area (see map) and wish to register with us, please call in to the surgery.   Our receptionists will give you a welcome pack with forms to complete and will request two forms of identification.  You will also need to complete a New Patient Questionnaire. If you are on regular medication, it is advisable to attach  your repeat prescription form.

You will then be given an appointment for a new patient medical with the Practice Nurse.  It is extremely important that you attend.

A named GP will be allocated to you however you can still see any of the GP's in the Practice.

You will also be offered a form to complete if you are a carer as this information will need to be recorded on the computer (with your permission).

Guide to GP Services

The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them.  You can download the guide below.
