REGISTER WITH THE PRACTICE?
As long as you live in our practice area (see map), please call in to the surgery
with your medical card. If you do not have your medical card, our receptionists
will give you another form to complete. You will also need to fully complete the
registration questionnaire. We need ALL the information requested.
You will then be given an appointment for a new
patient medical; it is extremely important that you attend.
You will be able to express a preference for the doctor of your choice, which
we will write on your questionnaire. We will endeavour to comply with your wishes
unless the preferred GP has reasonable grounds for refusing to provide services
to the patient; or the GP does not routinely perform the service in question.
HOW DO I...
OBTAIN A HOME VISIT?
Requests for house calls should be made before 10.00am and should be accompanied
by details of the problem. Calls after this time will be considered by the doctors
according to the degree of urgency.
Please remember that house calls are very time-consuming and are only available
for people who are unable to attend the surgery because of the severity of their
medical condition, and that patients should be seen at the surgery whenever possible.
Out-Of-Hours
Services between 6:30pm and 8:00am Monday to Friday
and all day Saturday, Sunday and bank holidays will be provided by the Hampshire Primary Care
Trust. You will be able to contact the out-of-hours duty personnel by ringing 023 9237 7921.

If you need advice on medical matters, this can be obtained from NHS Direct, a direct 24 hour helpline, on

HOW DO I...
OBTAIN A REPEAT PRESCRIPTION?
Repeat prescriptions are on a computerised system. Thus, when you require repeat medication, please bring your
computer request slip into reception, clearly marked with the items you require. Alternatively you may send your
requests to the surgery; in this case please enclose a stamped, addressed envelope in order that we may
post your prescription to you. You may also fax repeat prescription requests on 01329 282624 or email
your request to reception@gp-j82152.nhs.uk
