Practice Charter Standards

Patient Information

Why We Need Your Information

  • We ask you for information so that you can receive proper care and treatment.
  • We keep this information, together with details of your care, because it may be needed if we see you again.
  • We may use some of this information for other reasons, for example:
    • to ensure the health of the general public
    • to enable the NHS to run efficiently by making sure that services can meet patient needs in the future
    • to help staff review the care they provide and to make sure it is of the highest standard
    • to train and educate staff (you may choose whether or not to be involved personally)
    • to carry out research approved by the Local Research Ethics Committee (if anything to do with the research would involve you personally, you will be contacted to see if you are willing)
    • to investigate complaints or legal claims
  • Sometimes the law requires us to pass on information: for example, to notify a birth.
  • The NHS Central Register for England and Wales contains basic personal details of all patients registered with a general practitioner. The Register does not contain clinical information.
  • Everyone working for the NHS has a duty to keep information about you confidential
  • You may be receiving care from other people as well as the NHS. So that we can all work together for your benefit we may need to share some information about you.
  • We only ever use or pass on information about you if people have a genuine need for it in your and everyone’s interests.
  • Whenever we can we shall remove details which identify you.
  • The sharing of some types of very sensitive personal information is strictly controlled by law.
  • ANYONE WHO RECEIVES INFORMATION FROM US IS ALSO UNDER A DUTY TO KEEP IT CONFIDENTIAL.

You Have A Right Of Access To Your Health Records

Any requests for such access should be made IN WRITING to the Practice Manager.

Violent Or Abusive Patients

The practice has a no-tolerance policy to violence or abuse to any doctor, nurse or other member of staff. If patients are violent we will call the police and ask for the patient to be struck off, so that they are no longer registered here.

Please ensure that you treat all our staff and GPs with respect. Remember, we are always trying to do our best for you.

COMMENTS/COMPLAINTS

We operate a practice complaints procedure as part of the NHS system for dealing with complaints. Our system meets national criteria. Our practice complaints leaflet gives details of the procedure and is available from reception. Our aim is to give you the highest possible standard of service and we try to deal swiftly with any problems that may occur. Help us to help you.

Computer

We are registered in accordance with the provisions of the Data Protection Act.

  1. All patient details held on the computer will be kept confidential.
  2. Staff will not leave patients' details on the screen when leaving the computer unattended.

Freedom Of Information - Publication Scheme

The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the 'classes' of information the practice intends to routinely make available. This scheme is available from reception.
